Overview
This document was developed by the Interagency Working Group, is an attempt to give an overview of a root cause analysis for leaking LNS sachets. These documents can also be viewed in conjunction with the webinar presentation on LNS seal integrity available in the packaging section of this website. This is a living document as it can be updated in the future.
The two documents available are 1) an example of a completed fishbone diagram for a root cause analysis on a leaking sachet incident and 2) an excel spreadsheet (Annex 1) that gives a little extra information on the various root causes and how they can be corrected/eliminated. Overall, this is an example of a root cause analysis for a leaking sachet incident which suppliers can use for educational purposes when doing their own root cause analysis.
