Overview

This questionnaire is required to be filled in by each producer for the products that they are offering to UNICEF and Interagency Partners. This questionnaire is used as an initial screening tool to assess the suppliers quality system and product quality and safety. It gathers core information about the product as well as the controls in place at the manufacturer level to prevent the finished product from microbiological, chemical, and physical hazards to  ultimately ensure  food safety.

The questionnaire reviews the product specifications, technical requirements and quality and safety system currently in place. Manufacturers must supply nutritional information, information on microbiology and mycotoxins, chemical safety, primary packaging, patent/license, production process, monitoring plans, and finished product release protocols. This questionnaire is also a good tool for the preparation for an audit. It contains all the points approached during the MSF/UNICEF/WFP audit.
