Overview
This document, titled the Interagency Specialized Manufacturer Quality Questionnaire, is used to assess the quality management systems, production facilities, and monitoring controls at a manufacturer to prevent physical, chemical, and microbiological food safety hazards in nutrition products. The questionnaire applies to all specialised nutritional food suppliers and manufacturers and must be completed by each manufacturer, supplier, and subcontractor.
The questionnaire serves as a pre-audit tool for interagency quality and safety audits of specialised food manufacturers and suppliers. It covers general company information, quality management systems, Good Manufacturing and Good Hygiene Practices, quality control, storage, and transportation. Manufacturers must provide supporting documents listed in Annex I.
Suppliers and manufacturers should use this questionnaire to review and assess specialised food manufacturing site requirements to ensure the production of quality and safe nutrition products.
